Game Attendance

Each team must have five typed copies, one for each rink and one for you.

Game Roster 

Each team needs 24 typed copies, one for each rink and twenty for you.

Liability Form

Each team needs two forms per player, one for the league, one for you.

School Confirmation

Each player must be on school confirmation form; or the player must provide a copy of a school schedule or birth certificate accompanied by utility bill showing current address.
